HOW DO I APPLY FOR ADDITONAL VISA PAGES?
Please complete and sign and date application form DS-4085. Submit it along with your U.S. passport to the Consular Section during normal American Citizens Services hours.
There is no fee to have visa pages added to your valid U.S. passport. This service is usually completed in one day.
How do I change my name on my passport?
You will need to fill out form DS-5504 (if the change is within one year of passport issuance) or form DS-82 (if the change is more than one year after passport issuance) and bring evidence that describes the details of a name change or other change.
a. If your name has changed you must submit a marriage certificate or court order showing the change of name.
b. If there is a change or an error in your recently issued passport you must submit the appropriate evidence showing the correct information (i.e. birth certificate or marriage certificate).
c. All documents submitted for a name change or a data correct must bear the official seal of the issuing authority. Uncertified copies or notarized document cannot be accepted.
d. The DS-5504 does not require a fee. If your circumstances warrant use of form DS-82, you will need to pay all applicable fees. The embassy will return your re-issued U.S. passport as well as any documentary evidence you submitted.
