 HOW TO APPLY FOR A U.S. PASSPORT

U.S. Citizens soliciting a passport at U.S. Embassy San Jose need to apply in person at the Embassy during regular business hours.

Depending upon your situation, you will need to submit either DS-11 or DS-82. Please read the description carefully to determine your needs

1. DS-11: Application for a U.S. Passport

a. This form is to be used for first-time applicants, applicants whose previous passport was lost or stolen, applicants whose previous passport was issued before age 16, and applicants whose previous passport was issued more than 15 years ago.

b. You need to submit the following along with:

i. Proof of U.S. Citizenship
ii. Proof of Identity (i.e.: government issued photo identification)
iii. Two recent color photographs
iv. Fees

2. DS-82: Application for a U.S. Passport 

a. This form is to be used for applicants who:

i. Submit their most recent U.S. passport
ii. Were at least 16 years old when their most recent passport was issued
iii. Were issued their most recent passport less than 15 years ago
iv. Use the same name as their most recent passport or can submit proper documentation for their name change (marriage or court order).

b. What to submit with this form:

i. Most recent U.S. passport
ii. Marriage certificate or court order if your name has been legally changed
iii. Fee
iv. Two recent color photographs

APPLICANTS BORN IN THE UNITED STATES: 

Submit a previous US passport or certified birth certificate. A birth certificate must include your given name and surname, date and place of birth, date the birth record was filed, and the seal or other certification of the official custodian of such records.

APPLICANTS BORN OUTSIDE THE UNITED STATES:

Submit a Consular Report of Birth Abroad and provide the appropriate proof of identity. We may ask you to provide additional evidence to establish your claim to US citizenship.

PROOF OF IDENTITY

You must establish your identity to the satisfaction of the Consular Section of the United States Embassy.

You may submit items such as the following: identification containing your signature and physical description or a photograph that is a good likeness of you, previous US passport, Certificate of Naturalization, Certificate of Citizenship, driver’s license (not temporary or learner’s permit), or government (Federal, State, Municipal) employee identification card or pass. Temporary or altered documents are not acceptable. When necessary, we may ask you to provide additional evidence to establish your identity.
