Replacing a Lost or Stolen Passport

You will need to come to the American Citizen Services Unit of the U.S. Embassy during the hours of operation and fill out the DS-64 form, Statement Regarding Lost or Stolen Passport. At that time, you can also fill out the DS-11 application for a new passport. Please bring the police report, if you have one.
